The reimbursement will be made according to the established percentage, and up to the limit of the insured capital, once the client provides the documentation necessary to prove that you have paid the bill, as well as the prescription document from an ophthalmologist.

Specifically, it will be essential to provide the following documents:

• Duly completed refund request form.
• Original invoice broken down by amounts/concepts.
• Medical prescription from an ophthalmologist, in the name of the insured.
• All documents always original.

Once all the documentation has been received and the appropriate checks have been carried out, within 10 working days the refund will be made the corresponding amount.
